DIGNITY HEALTH ARIZONA PREFERRED/PREMIER PLANS
Dignity Health employees who live and work in Arizona
Dignity Health Preferred Network (Tier 1)
Tier 1 Mental Health and Substance Abuse providers (United Behavioral Health Network) Banner Health is in-network if contracted through United Behavioral Network for substance abuse or mental health treatment. Mayo Providers are out of network for mental health or substance abuse even if listed in the United Behavioral Health Network
Search the UnitedHealthcare (UHC) Choice Plus Network (Tier 2). Banner Health facilities, Urgent Care and hospitals will be considered out of network for the Dignity Health Arizona Preferred and Dignity Health Arizona Premier plans, even though they are listed in the UHC Choice Plus Network directory. Mayo Clinic Hospital and Physicians are no longer an in-network provider for Preferred or Premier Plans and will process at the out of network benefit, even though they will list in the UHC Choice Plus Network Directory.

A provider at times may no longer participate in the network, or their tier status may change resulting in different cost sharing. Soon after the provider changes their status, out-of-network penalties or increased cost sharing will result. Please call the customer service phone number on your ID card to learn more about the participating providers in your network(s) and the implications, including financial, if you decide to receive your care from non-participating providers.
